The forms in this section are intended for new and potential clients of our office, or those that are taking advantage of our free consultation policy.  As we do not charge for consultations, we kindly ask that you arrive at your appointment prepared, so that your time and ours' may be used efficiently.

Divorce | Dissolution Intake

This intake form is for our potential divorce and/or dissolution clients.   This intake requires brief summaries of assets, debts, income, and child related information.

Child Custody & Parenting Time Intake

This intake form is for our potential family law clients regarding issues of custody, shared parenting, parenting time, and other child custody related matters.

Criminal | Traffic Defense

This intake form is for potential clients facing criminal charges or traffic citations.

Current Butler County Divorce &| Dissolution Clients:

DR602 Affidavits

Shortly after a divorce is filed, and before a dissolution may be filed, each spouse must complete, sign before a notary, and exchange DR602 affidavits.  Since it is impossible to determine if an agreement is a fair and equitable division or allocation of all assets and debts, without knowing everything that each party owns, the required Butler County Domestic Relations Court 602 forms are required as part of each parties' mandatory disclosures.  Not only are these required by the Court, but the timely completion of these forms also ensures that your case proceeds efficiently and helps to ensure that we know everything that exists. 

DR602A | Income Affidavit

Requires the reporting of all income from all sources. Estimate Spouse’s to the best of your knowledge. If unknown, but you are aware of a range or estimate, state your estimate or range and that it is an estimation by including “EST.”

DR602B | Property Affidavit

All property and debts known to you, regardless of which spouse owns the asset, or is obligated on a debt, and regardless of whether you want to make a claim to the specific item, or not. Only write “NONE,” If you are certain that none exists. By way of illustration, if each spouse is fully aware of his/her own assets, accounts, debts and is fully aware of those of his/her spouse, each party’s DR602B Property Affidavit would mirror the other party's DR602B Property Affidavit.

DR602C | Expense Affidavit

All expenses for your household, your current residence in which you reside.
